Fluoroscopy Guided Pain Injections

Overview

What Are Fluoroscopy Guided Pain Injections?

Fluoroscopy guided pain injections use continuous low-dose X-ray imaging to help clinicians place medication with precision. This enables accurate delivery of corticosteroids and/or local anaesthetic into areas such as nerve roots, facet joints, sacroiliac joints or the epidural space.

Chronic back pain is one of the most common causes of disability in the UK. This makes precise, targeted treatment an important option for long-term symptom relief.

How Fluoroscopy Guided Pain Injections Work

Using fluoroscopy, the consultant guides a fine needle to the exact location of the pain. Sometimes, contrast dye is used to confirm positioning. Medication is then injected directly into the targeted structure.

  • Corticosteroids reduce inflammation and may offer longer-term relief
  • Local anaesthetic provides short-term relief and can support diagnosis
  • Some injections combine both for diagnostic and therapeutic benefit

What Fluoroscopy Guided Pain Injections Can Be Used For

These injections may help manage:

  • Chronic back, neck and facet joint pain
  • Radiculopathy (such as sciatica)
  • Herniated or bulging discs
  • Spinal stenosis
  • Facet joint arthritis
  • Sacroiliac joint dysfunction
  • Nerve entrapments and neuropathic pain
  • Post-surgical spinal pain
  • Inflammatory arthritis

What to Expect When Receiving Fluoroscopy Guided Pain Injections in Sheffield and Leeds

At your appointment, a team of highly trained healthcare professionals will welcome you and take time to understand how your symptoms affect your daily life. Your consultant will review previous treatments and discuss whether a steroid injection, radiofrequency treatment or local anaesthetic block is appropriate.

Before Your Appointment
Your consultant will check your medical history and any medications you are taking. This is a good opportunity to mention new or changing symptoms. Once approved for treatment, arrange transport for the return journey, as you will not be able to drive.

During Your Appointment
You’ll have an X-ray to identify the correct injection site. The consultant will mark the area and clean the skin before positioning the needle under live fluoroscopy. Once the needle is precisely placed, medication is delivered, and a dressing is applied.

After Your Appointment
Most people stay at the clinic for monitoring for 1–3 hours. Our team will check your blood pressure, assess for numbness or weakness, and ensure you feel well before you leave. Someone should drive you home and, ideally, stay with you for a short time afterwards. Mild soreness or bruising is common.

Risks and Complications of Fluoroscopy Guided Pain Injections

Serious complications are rare (1:10,000). These include spinal infection/ bleeding/ nerve or spinal injuries. Injections are only for ongoing pain that hasn't responded to simpler measures for example pain killers, a period of rest or physio.  Possible complications include:

  • Infection at the injection site
  • Bleeding or bruising
  • Temporary increase in pain
  • Allergic reaction to contrast dye or local anaesthetic
  • Temporary numbness or weakness
  • Leg weakness requiring short-term observation

Your consultant will explain these risks before treatment.

How to Recover Following Fluoroscopy Guided Pain Injections

Most people can resume light activities within a day or two. Speak to your consultant before returning to strenuous exercise or heavy lifting.

Many patients return to work the following day, although lighter duties may sometimes be recommended. Discomfort or bruising usually settles after a few days.
