Landmark guided pain injections use anatomical landmarks on the body to guide needle placement without the use of imaging. The aim is to deliver local anaesthetic and/or corticosteroids directly into targeted areas to reduce inflammation and ease pain. Consultants may also use them to help diagnose the underlying source of symptoms.

Corticosteroid injections can provide relief that lasts from several weeks to several months, depending on the condition.

How Landmark Guided Pain Injections Work

A landmark guided pain injection relies on expert anatomical knowledge and clinical experience. A healthcare professional identifies surface landmarks to guide the needle to the correct area. Once positioned, medication is injected into the intended structure – whether that’s nerve roots, facet joints, sacroiliac joints, soft tissues or specific areas of the spine.

Local anaesthetic temporarily blocks nerve signals, easing pain for a few hours, while corticosteroids reduce inflammation and can offer longer-term relief over several weeks or months.

What to Expect When Receiving Landmark Guided Pain Injections

At your appointment, our team will take time to understand how your pain affects you and discuss suitable options. You’ll be treated as an outpatient and can go home the same day.

Before Your Appointment
Your consultant will review your medical history, previous treatments and current symptoms. If rest, physiotherapy or oral medication haven’t eased your pain, they may recommend landmark guided pain injections. You’ll be advised about any activities to avoid beforehand and whether you should pause certain medications.

It’s sensible to arrange transport home, as you should avoid driving immediately after the procedure.

During Your Appointment
A consultant performs the injection in a clinical outpatient setting. After identifying the anatomical landmarks, they clean the skin and place the needle into the targeted area. You may feel some brief pressure or discomfort, but most procedures take only a few minutes.

After Your Appointment
You’ll stay with us for a short observation period so we can make sure you feel comfortable before going home. Mild soreness or swelling at the injection site is common. Your consultant will advise when you can safely return to normal activities.

Risks and Complications of Landmark Pain Injections

Landmark guided injections are generally considered safe. Most side effects are mild, such as temporary soreness or swelling. Serious complications are uncommon, and your clinician will discuss any specific risks with you before treatment.

How to Recover Following Landmark Guided Pain Injections

You may feel immediate relief from the local anaesthetic, although this is short-term. Steroid medication usually begins to work after a few days and can last for weeks or months.

Most people can return to light activities within 24–48 hours. You should avoid strenuous exercise and heavy lifting for the first couple of days.
